CLIENT AGREEMENT
Welcome! Thank you for making the decision to work with me, Elissa Lowenstern.
This document sets out the rights and responsibilities we are agreeing to as practitioner and client. Please take a moment to read it, as it sets out the terms of our relationship so that we may both benefit from clear boundaries and knowing what to expect from each other.
When you engage my services, you agree that you are over the age of 18 and willing to be bound by these terms and conditions.
Disclaimer for Services
I am a qualified Kinesiologist, Psych-K® practitioner, and am trained in Brain Gym and Brain Integration. I have run my own holistic wellbeing clinic since 2008 and have extensive experience working in a multidisciplinary clinic environment.
I am passionate about helping people improve their lives, having personally overcome great odds in my own life. This has given me a wealth of grounded grass roots experience.
I am committed to holding a professional, sacred space for us to experience a mutually beneficial working relationship and take best practice very seriously. This includes keeping my skills up to date with regular ongoing professional development, staying in touch with the latest research, and taking all reasonable care in my work and client relationships (as required by me pursuant to the general code of conduct).
My intention in offering these services and products is to help you shift the stress and limiting beliefs that are holding you back from living a happy and confident life.
Your responsibility
With energetic work, it can, at times, feel like nothing is happening, so you will need to relax into the process during the session, while at the same time understanding that between sessions you will need to do the work yourself. You will get the best results if you are open and ready to create the change that you want, take the time to integrate and process your emotions, and follow through on any work that you agree to do. Patience and perseverance are a big part of taking ownership of your journey and creating sustainable change.
If you are working with a different health practitioner, it is important that you inform me and continue any treatment plans. I am always happy to work collaboratively as part of your holistic health care team.
Important Information
I ask that you be reasonable, and not have unrealistic expectations about how much we can cover in a session. I may suggest that we make note of something that you would like to work on and return to it in our next appointment if we don’t have sufficient time to address it effectively.
My work is not a substitute for medical advice or diagnosis, mental health support, therapy or counselling. If any concerns arise during our work together, I strongly recommend you seek advice from a qualified professional or support service. It is always important to conduct your own research, seek appropriate advice and decide whether something is right for you before making major changes to your lifestyle.
Further, I am not qualified to give you advice on prescription drugs. You must seek advice from a suitably qualified medical health practitioner.
Privacy Policy
Please ensure that you have read and understand my privacy policy. I am passionate and committed to protecting your privacy and personal information. If you have any questions or concerns about how your personal information is being handled, please do not hesitate to contact me.
Payment Terms
Payment for all online consultations and programs is to be made in advance using Paypal. Face to face consultations may be paid at the end of the session via direct debit, credit card or cash.
I do not have access to your account details. Any financial information you enter is stored under Paypal’s security policy.
Delivery
Intake forms must be filled out prior to your first consultation. This is available through my website.
Online consultations are held using Zoom video conferencing software. You are provided with the session link when you book through Acuity. Please make sure that Zoom is installed, and you have checked your set up in advance so that you are ready to go at the start of the session. You will need to ensure that you are in a comfortable, private spot where you won’t be disturbed.
Appointment Policies
I require at least 24 hours’ notice to reschedule or cancel an appointment unless there is an emergency beyond your control, in which case you must contact me as soon as possible to explain the circumstances. Missed appointments or those which are cancelled or rescheduled with less than 24 hours’ notice will forfeit the consultation fee, as I have held that space open in my calendar for you.
Please ensure that you are on time, as it is not possible to extend the duration of the consultation if you are late. We will still finish at the scheduled time.
Refund Policy
I want to be sure you are happy with your decision to work with me. If you cancel your booking more than 24 hours before the scheduled appointment time, I will provide you with a full refund.
Dispute Resolution & Jurisdiction
If you have any concerns, issues or complaints arising out of our sessions together, you agree to communicate with me with the intention of making a genuine effort to seek a win/win solution and trying to resolve the dispute in good faith through negotiation and discussion. Please email me at elissa@elissalowenstern.com and expect a response within 2 business days.
If we are unable to resolve the matter, we can proceed to mediation. Details are set out in my Terms & Conditions document on my website.
